Posaconazole is a broad-spectrum triazole active against various yeasts and molds, including Aspergillus, Candida, Fusarium, and Zygomycetes. Compared to other antifungals, posaconazole is a significantly more potent inhibitor of sterol 14-alpha demethylase. Posaconazole is the most advanced candidate for the treatment of Chagas disease. Posaconazole has entered in a phase II clinical trial in the treatment of mycoses.

Function
Posaconazole exerts its antifungal activity by inhibiting ergosterol biosynthesis.Ergosterol is an important component necessary for fungi to maintain the structural integrity of cell membranes and to exert some membrane-related protein functions, and is also a necessary trace substance in fungal cell cycle.Posaconazole blocked ergosterol synthesis by inhibiting fungal pigment P45014a-demethylase, resulting in accumulation of 14a-methylsterol, etc. The degree of blockage was related to the sensitivity of the test strain and dosage of posaconazole.
Application
1.Posaconazole is used for the prophylaxis of invasive Aspergillus and Candida infections in patients who are at high risk of developing these infections due to being severely immunocompromised, such as HSCT recipients with GVHD or those with hematologic malignancies with prolonged neutropenia from chemotherapy.
2.Posaconazole is also used for the treatment of oropharyngeal candidiasis (OPC), including OPC refractory to itraconazole and/or fluconazole therapy.
3.Posaconazole is also used to treat invasive infections by Candida, Mucor, and Aspergillus species in severely immunocompromised patients.
